Preoperative assessment of bleeding risk consists of administering a structured bleeding questionnaire which, in conjunction with physical examination, will guide laboratory testing. In the vast majority of cases a positive bleeding history will require referral for specialist assessment and management. Conversely, a negative initial screen and examination may exclude patients from further evaluation. Routine coagulation screening prior to surgery or other invasive procedures to predict postoperative bleeding in unselected patients is not recommended

Permissive Hypotension [StatPearls]

Permissive hypotension refers to a deliberate medical strategy, particularly in emergency and critical care settings, where blood pressure is allowed to remain lower than normal to minimize the risk of further injury or complications. This approach is often used in situations where significant bleeding or trauma has occurred, ensuring adequate vasoconstriction and organ perfusion and preventing adverse effects during resuscitation.
The procedure is beneficial in hemodynamically unstable patients. However, the impact of demographics and prior history on the effectiveness of permissive hypotension is unknown, and its efficacy in blunt trauma cases is variable. Understanding the effects of excessive or insufficient volume in trauma care requires a profound grasp of cardiovascular physiology relating to systemic and peripheral circulations.
